Important: A valid BLS certification is a prerequisite for the ACLS and PALS courses.

I understand and agree to the following:

• The Heart & Stroke course(s) must be taken in Alberta
• Eligible expenses for reimbursement, including GST, are course fees and materials up to the maximum amount per course ($100 BLS and $300 for PALS or ACLS)
• Proof of purchase and completion are required to be eligible for reimbursement
• Ineligible expenses for reimbursement include cancellation or rebooking fees, food, and parking
• Funding is offered on a first-come, first-served basis, and is not available if you have been or will be paid by another source for the course/certification
